Hypertension is high blood pressure in your arteries. It is recorded as two numbers, e.g. 120/80. The top number (systolic) is the pressure in the arteries when the heart contracts. The bottom number (diastolic) is the pressure in the arteries when the heart rests between each heartbeat. A reading of 150/90 mmHg or above indicates high blood pressure.

A lot of patients ignore their blood pressure as high blood pressure itself has no symptoms. It is however a major risk factor for serious health problems like heart attacks, strokes, and heart failure.

In general, the higher your blood pressure, the greater the health risk. But, high blood pressure is just one of several possible risk factors for developing a cardiovascular disease. Other risk factors include smoking, alcohol, high cholesterol, diabetes, age, being male, family history, sedentary lifestyle, ethnic origin and obesity, among others.

Apart from taking your prescribed medication, it is important to make changes to your lifestyle where required, e.g.: lose weight, stop smoking, exercise, eat a healthy diet, reduce salt and caffeine, limit alcohol intake.

Novel Practice Hypertension Service

We have introduced a practice hypertension service focusing on preventing complications and achieving the best possible care for our patients.

Given that the GHS does not currently offer a recall system, we wanted to innovate and create a system whereby we will be informing our patients about what check-ups they need to have every year (see table below).

You will be called to see the nurse for an annual review. This is called pre-diabetes and can be identified by blood sugar (glucose) tests. Technically, pre-diabetes may result from one of three conditions called Impaired Fasting Glycaemia, Impaired Glucose Tolerance and Increased Glycated Haemoglobin. However, the effect is the same – it means raised blood sugar and a higher risk of getting diabetes and heart disease.

Out of 100 people with prediabetes, around 33 will develop type 2 diabetes, and about 70 will progress to diabetes within a few years if they don’t make lifestyle changes. However, it is not a definite outcome, as healthy lifestyle changes like increased physical activity, a healthier diet, and weight loss can delay or even prevent the progression to diabetes in most people.

How to prevent type 2 diabetes

If you have pre-diabetes, you can lower your risk for developing type 2 diabetes by adopting healthier lifestyle choices:

1. Eat healthy foods. A diet high in fruits, vegetables, nuts, whole grains and olive oil is associated with a lower risk of diabetes. Choose foods low in fat and calories and high in fiber. Eat a variety of foods to help you achieve your goals without compromising taste or nutrition.
2. Be more active. Physical activity helps you control your weight, uses up sugar for energy and helps the body use insulin more effectively. Aim for at least 150 minutes of moderate or 75 minutes of vigorous aerobic activity a week, or a combination of moderate and vigorous exercise.
3. Lose excess weight. If you’re overweight, losing just 5% to 7% of your body weight can significantly reduce the risk of type 2 diabetes. To keep your weight in a healthy range, focus on permanent changes to your eating and exercise habits.
4. Stop smoking. Stopping smoking can improve the way insulin works, improving your blood sugar level.
5. Take medications as needed. If you’re at high risk of diabetes, your healthcare provider might recommend metformin (Glumetza). There are several causes of vertigo, but one common cause with increasing age is benign paroxysmal positional vertigo (BPPV).

BPPV is almost always triggered by a change in your head’s position. Some people may notice symptoms when lying down or sitting up in bed. Others might notice symptoms when they tilt their head back or to the side. These symptoms often worsen with age due to normal wear and tear of the inner ear structures.

How common is BPPV?

Benign paroxysmal positional vertigo is the most common inner ear disorder. A modified Epley manoeuvre can be performed by the patient at home.

It is important for elderly patients experiencing a persistent episode of vertigo that is not related to posture or head tilt, and who are unable to walk in a straight line, to see their personal doctor to rule out a cerebellar stroke.

[148

post-3260","post","type-post","status-publish","format-standard","has-post-thumbnail","hentry","category-physiotherapy
["GET

Lower back pain is a very common condition involving the muscles, nerves, and bones of the back, which is often caused by muscle or ligament strains, but can also stem from disk injuries, arthritis, osteoporosis, or structural problems.

As we age, wear and tear causes degenerative changes in the joints, resulting in pain and sometimes swelling. Some patients may also develop inflammation of the tendons or damage to the muscles that hold your joints together.

Treatment

The good news is that patients usually do not require surgery; they just need the right medication and intensive physiotherapy. Physiotherapy can also be used to treat patients with sciatica (i.e. lower back pain accompanied by shooting pain in the back of the legs). An MRI scan is only necessary if physiotherapy is unsuccessful.

The principle of physiotherapy is to strengthen your muscles, thus stabilising your joints and reducing wear and tear, inflammation and ultimately pain.

Home Remedies & Self-Care

Rest: Initial rest can help, followed by a gradual return to light activity.

Heat/Cold Therapy: Apply a cold pack, then switch to heat to relax muscles.

Over-the-Counter Pain Relievers: Medications like ibuprofen or acetaminophen can help manage pain.

Gentle Exercise: Low-impact activities such as walking, swimming, or

Pilates can improve symptoms and prevent future episodes.

Good Posture: Maintain good posture and use lumbar support when sitting.

[148

post-3258","post","type-post","status-publish","format-standard","has-post-thumbnail","hentry","category-physiotherapy
["GET

About COPD

Chronic obstructive pulmonary disease (COPD) is an umbrella term for chronic bronchitis and emphysema, lung diseases that cause airflow obstruction, leading to shortness of breath and a chronic cough.

COPD is usually caused by smoking, and patients with COPD are therefore at an increased risk of cardiovascular disease. Patients often experience exacerbations, sometimes requiring antibiotics and oral steroids. Therefore, it is important for patients with frequent exacerbations to keep a rescue pack of antibiotics and steroids at home (please discuss this with your doctor). Signs that you are having a COPD exacerbation include feeling more breathless, an increased amount of sputum or a change in its colour, and fatigue. A visual guide to this, in the form of a COPD action plan, is attached below and should be kept for reference at all times.

Novel Practice COPD Severe

We recently introduced a practice COPD service in order achieve the best possible care for our patients. It is crucial that patients with COPD have a yearly check up for their asthma, even if it is well controlled.

Given there is no current recall system through the GHS (General Healthcare System) we wanted to innovate by creating a system whereby we offer annual reviews. After the annual review by the nurse, you will see the doctor on the same day. Please make sure you have your inhaler and preferably a printed COPD action plan with you (see below).

Annual Review Appointments

Your personal doctor will usually invite you for an annual COPD review. However, as it is not possible to arrange your first appointment through the GHS, you will need to book it yourself. Following this, you will be invited for a follow-up appointment. If you have not been invited for a recall appointment, you can book an appointment yourself.

If you have severe COPD, you need to go for an asthma review more often.

How to prevent COPD flare-ups

Create and follow a COPD action plan (see below)

Take your medicines as prescribed

Stay up to date with vaccines for the flu, COVID-19, and pneumonia

Check with your Personal Doctor that you are using your inhaler correctly

Take inhalers everywhere with you

Keep a diary of symptoms

Consider getting lifestyle/smoking cessation advice

Create an action plan to manage your COPD

Those at risk of having an exacerbation of COPD are encouraged to create a self-management action plan with the help of their doctor, which will help them manage their COPD and recognise when their symptoms are worsening.

The plan includes information on:

your medicines and how to take them

how to control your symptoms

what to do if you have an attack

how to know if your symptoms are getting worse and what to do

emergency contact information

[131

post-3138","post","type-post","status-publish","format-standard","has-post-thumbnail","hentry","category-chronic-disease
["GET
